Ugh, yes, I know exactly what pattern your're talking about. And they almost always use "hermaphrodite" instead of "intersex" and the author frequently seems to think they've invented the notion, and not pay attention to the existence of trans or intersex people as something that might possibly inform their characters. It's just this magical fantasy of what the cis author thinks it would be like to be Hermaphroditus. I'm very intentionally looking for examples of people doing things that are actually different.

And huh, you're right. I don't recall having ever seen a trans guy in a fantasy novel, despite the large number of girls-living-as-boys stories I've read. (The fanfiction I itch to write about Alan of Trebond... there is a lot of it.)
